SSI / NASDS Lost C Cards Forms

If you need a replacement card, there are two simple ways you can get one:

1. If you have proof of certification, your logbook, wall certificate or other identification, or we can verify your certification on file with SSI for your SSI or NASDS certification, we can process your replacement card in the store for $25.
Bring in your proof of certification or we need:
Name at the time of Certification

First, Last and Middle Initial.
Year Certified
Social Security #
Date of Birth
Agency Certified
Photo (we can take a digital photo of you at the dive store, or you can bring in your own)

2. If you are in a hurry, and need a card right away, you can order a card directly through SSI. Just print out the Replacement Card Form*, fill it out, and mail it to SSI Headquarters.
